29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about February 4, 2016, during the maintenance/repair of an HVAC unit, at the job site located at 19071 Old Detroit Road, Suite 8, Rocky River, Ohio, 44116, the employer did not ensure that employees working on an ice coated roof, at a height of approximately 11-12 feet above the ground, were protected from a fall hazard by utilizing personal protective equipment.

Employees were working on top of an ice coated commercial roof.
