Building 1 and building 3, during progress of work at the second, third and fourth floor balconies, railing were removed and not replaced, exposing employees to a fall hazard, on or about 02/04/2016. b. Building 1 fourth floor, the top rail at the elevator shaft was removed and not replaced, exposing employees to a fall hazard, on or about 02/04/2016. c. Building 1 third and fourth floor porch at the west side, an employee was exposed to a fall hazard during guardrail installation, on or about 02/04/2016.
