Located at 6335 Refugee Road in Canal Winchester, Ohio: On or about February 3, 2016, an employee was operating a Dewalt angle grinder without the use of approved safety glasses, exposing the employee to a struck by hazard.

29 CFR 1926.404(b)(1)(i): General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.  a. Located at 6335 Refugee Road in Canal Winchester, Ohio: On or about February 3, 2016, employees were using electrical equipment, such as an angle grinder, circular saw, and battery chargers, which were attached by extension cords to a Predator 6500 watt portable generator. The electrical circuit was not protected by the use of a ground fault circuit interrupter, exposing employees to an electrocution hazard.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide.   a. Located at 6335 Refugee Road in Canal Winchester, Ohio: On or about February 2, 2016, employees were working from the mid and upper railings of four JLG scissor lifts. The railings did not meet the requirements of an acceptable scaffold platform, thereby exposing employees to an approximately thirteen foot fall hazard.

Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).   a.
