29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a. In the basket of the aerial lift, fall protection was not provided to an employee replacing a vinyl sign on the shopping center's sign tower along the roadway, exposing the employee to a 15 foot fall hazard, on or about 2/3/2016.
