29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a. On or about February 2, 2016, employees walking around on the roof to access the ladder were not protected from falling 12 feet to the ground below by the use of a guardrail system, safety net system or personal fall arrest system.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    a. On or about February 2, 2016, front of building, ground level, employees were using a Werner 12-foot step ladder to gain access and exit from the roof.  The ladder was not capable of being extended 3 feet past the landing and was not secured and there was no grasping device to assist employees in mounting and dismounting the ladder.  Employees were being exposed to a 12 feet fall hazard.

On or about February 2, 2016, front of building, ground level, employees were using a Werner 12-foot step ladder in the closed position to gain access and exit from the roof.  They were being exposed to a 12 feet fall hazard.
