29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Laminator area: The inrunning nip point between the heat drum and lay on roller of the Lembo laminator was not properly guarded in that an employee sustained a severe arm injury when his hand and arm were caught between the two rollers, on or about February 2, 2016.  b) Laminator area: The inrunning nip points between the rollers and belt on the trim puller were not properly guarded, as observed on or about February 5, 2016.  Abatement Certification Required.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  a) Laminator area: A cover on the rear of the laminator control cabinet was missing exposing the energized electrical conductors and components to contact, as observed on or about February 5, 2016.  Abatement Observed During Inspection.
