29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: A.) On or about January 29, 2016 at 31-89 123rd Street, Flushing, NY 11354 The employer failed to report OSHA the in-patient hospitalization of an employee within twenty-four (24) hours. An in-patient hospitalization was not reported to OSHA until February 2, 2016. ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
