29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.  On or about February 2, 2016, on the north west side of a new residential construction, an employee working beneath an employee applying stucco from a five buck scaffold was not wearing a hard hat, exposing the employee to the hazard of being struck by falling debris.

29 CFR 1926.451(b)(3): Where guardrails were not installed along the front edge of the platform or all employees were not protected from falling with the use of personal fall arrest systems, the front edge of the scaffold platform during plastering and lathing operations:   a.   On or about February 2, 2016 on the south west side of a new construction residential house, an employee applying stucco material to the facing of the house was not protected from a 20 feet 6 inch fall hazard by the use of a personal fall arrest system.  b.   On or about February 2, 2016 on the north west side of a new construction residential house, an employee applying stucco material to the facing of the house was not protected from a 13 feet 7 inch fall hazard by the use of a personal fall arrest system.

On or about February 2, 2015 on the south west side of a new residential house, an employee applying stucco to the surface of the house was using a scaffold that that had platforms that were not at least 18 inches wide, exposing employees to a 20 feet 6 inch fall hazard.  b.  On or about February 2, 2016 on the north west side of a new residential house, an employee applying stucco to the surface of the house was using a scaffold that that had platforms that were not at least 18 inches wide, exposing employees to a 13 feet 7 inch fall hazard.

On or about February 2, 2016 on the west side of a new residential house, the employer did not ensure employees applying stucco to the face of the house were using a scaffold that was inspected prior to its use for it's structural integrity, exposing employees to fall hazards.

29 CFR 1926.451(g)(1)(v): Each employee on a walkway located within a scaffold was not protected by a guardrail system (with a minimum 200 pound toprail capacity) installed within 9.5 inches (24.1 cm) of and along at least one side of the walkway:  a.  On or about February 2, 2015 on the south west side of a new residential house, an employee applying stucco to the surface of the house was using a scaffold that that had guardrails installed with a 12 inch opening along the walk board on one side, exposing employees to a fall hazard.

On or about February 2, 2016 on the north west side of a new residential house, employees working on a five buck frame scaffold did not use toeboards on the edges, exposing employees working underneath to struck-by hazards.
