29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Foam Department, on or about 2/5/16: Edge Sweets horizontal saw was not equipped with guarding to prevent accidental prevent contact with the saw during operation. b) Foam Department, on or about 2/5/16: Edge Sweets horizontal saw was not equipped with guarding to prevent access to the carousal table and saw during operation. c) Foam Department, on or about 2/23/16: Contour Saw blade was not provided with guarding to prevent contact. d) Flexographic Press Area, on or about 2/5/16: Guards were not provided over the in-running nip points of the belts and rollers on the stacker conveyor. e) Slitter Area, on or about 2/24/16: SR Bandsaw was not provided with adequate guarding over the in-running nip points of the blade guides.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Foam Department, on or about 2/5/16: #4 Vertical bandsaw was not adequately guarded to prevent the operator from point-of-operation hazards in that the guard was positioned approximately 13-inches above the material being cut. b) Foam Department, on or about 2/5/16: Rockwell/Delta vertical bandsaw was not guarded to prevent the operator from point-of-operation hazards. The saw is used to cut foam material 1/2-inch thick thereby exposing the operator to 9-1/2 inches of unused blade. c) Slow Speed Converting Area, on or about 2/5/16: S & S Corrugated Paper Machine #28 was not provided with adequate point-of-operation guarding in that the opening above the table measured 5-3/4 inches and the material being cut 1/4-inch thick. d) Slow Speed Converting Area, on or about 2/5/16: Universal 4 Bar Slitter was not provided with adequate point-of-operation guarding in that the opening above the table measured 5-inches and the material being cut 1/4-inch thick. e) Slow Speed Converting Area, on or about 2/5/16: Universal 6 Bar Slitter was not provided with adequate point-of-operation guarding in that the opening above the table measured 3-inches and the material being cut 1/4-inch thick. f) Slow Speed Converting Area, on or about 2/5/16: Partition Slotter machine was not provided with point of operation guarding.
