29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. At the establishment in the cleaning department, employees operating the 15.75" tall shop made pneumatic punch press were exposed to crushed-by and amputation hazards: a) The punch press was not equipped with a barrier type guard at the point of operation of the punch area, and b) The punch press foot pedal did not have a guard over the foot pedal to prevent accidental actuation of the press.
