29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment would be isolated from the energy source and rendered inoperative:      As of 01/20/2016 the employer failed to establish an effective program for the control of hazardous energy at the facility.  Energy sources used at the facility include; but are not limited to, electric, hydraulic and pneumatic energy.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     The employer failed to ensure that various equipment or machinery used at the site was effectively guarded.  The unguarded equipment includes; but is not limited to, the following:            a. The Millport (vertical) drill and boring mill was unguarded. Employees are potentially exposed to rotating parts, struck- by hazards or flying chips and sparks.        b. The Cincinnati (vertical) drill and boring mill was unguarded. Employees are potentially exposed to rotating parts, struck- by hazards or flying chips and sparks             c. The rotating chuck of the five of the company's lathe located in machine shop area did not have a chip/chuck guarding.  Employees are potentially exposed to an entanglement hazards.
