29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. A) North side of building, on or about February 5, 2016, two employees were working from the fifth level of a six level scaffold that did not have a mid-rail as part of the guardrail system, exposing the employees to a 30 foot fall hazard. B) North side of building, on or about February 5, 2016, an employee was working from the third level of a four level scaffold that did not have a guardrail system on the open side of the scaffold platform, exposing the employee to an 18 foot fall hazard.
