OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to crushed-by and caught-in hazards when operating an ironworker. a) Production Area, Lower Shop, 300 Sly Street, Luzerne, PA - The Peddinghaus Universal Steelworker Combined Shearing and Punching Machine Model 210 Super 16 (Serial No. 4408958180002) had a broken shroud that left the foot pedal unguarded exposing employees to crushed-by and caught-in hazards from unintended activation of the machine.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replaced. a) Production Area, Upper Shop, 300 Sly Street, Luzerne, PA - A cable (welding lead) connecting an electrode holder (stinger) and a shielded metal arc welder (SMAW) was used daily, had damaged insulation, exposed bare conductors, and lay across metal tracks exposing employees to an electrical shock hazard, on or about February 5, 2016.
