29 CFR 1910.110(f)(4)(i): The quantity of LP-Gas stored shall not exceed 300 pounds (approximately 2,550 cubic feet in vapor form) except as provided in subparagraph (5) of this paragraph.    The Company created a fire hazard for employees inside the facility by storing 31 full, 23 lb.  LPG cylinders, a total of 713 lbs, in two storage racks unprotected by physical damage.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).    a.  Prior to November 20, 2015, two employees who operated forklifts on a daily basis in the loading area and warehouse had not been trained as required by paragraph (l).    b.  Employee who operates a Yale power walkie on at least a monthly basis had not trained as required by paragraph (l).

29 CFR 1910.178(n)(4): The driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing.    On or about November 20, 2015, a Forklift Operator operating a forklift, was leaving the Loading Area and moving through the entrance into the Warehouse Area did not slow down and sound his horn and an employee who was walking on the right side in the aisle way and was struck by the forklift operator operating the forklift.

You must record on the OSHA 300 Log the recordable injuries and illnesses of all employees on your payroll, whether they are labor, executive, hourly, salary, part-time, seasonal, or migrant workers. 29 CFR 1910.36(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors.    In the Warehouse North Exit door the exit door had a hasp on it which would require an employee to have special knowledge before exiting the door.

29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked.    In the Warehouse Area, the Company had not marked their aisle ways in this area.
