29 CFR 1910.147(c)(6)(i):     The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.    Throughout Facility- On or about February 3, 2016, the employer had not performed periodic inspections to ensure that documented lockout/tagout procedures were being followed by authorized employees.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    Production Department - On or about February 3, 2016, the employer failed to guard the ingoing nip points, in that the rollers on the Braner Slitter w/Turret Head, serial number SL-11691 was accessible, thereby exposing employees to rotating parts.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.    Throughout Facility - Recordkeeping - On or about February 3, 2016, the employer did not include what directly injured or made the employee ill in the 2015 OSHA log.
