29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a) Front of Building - On or about February 3, 2016, employees working on a second level unfinished wood deck lacked head protection when working beneath employees using power tools and other tools to build a third level covering for the second floor deck.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  (a) Front of Building - On or about February 3, 2016 employees were not wearing eye or face protection while using  pneumatic air guns loaded with 16D 3 and 1/2 inch nails to attach a wood plate against the building.

29 CFR 1926.304(d): Each portable, power-driven circular saw withdrawn from the work did not automatically and instantly return the lower guard to the covering position of the saw.  (a) Front of Building - On or about February 3, 2016 employee using the Makita circular saw was cutting wood with the guard wedged in the open position thereby preventing it from automatically and instantly returning to the covered position of the saw.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Front of Building - On or about February 3, 2016 an employee building an elevated wood deck without fall protection was exposed to a 12 foot 3 inch fall hazard.    (b) Front of Building - On or about February 3, 2016 two employees were exposed to a 23 foot and 6 inch fall hazard while mounting a wood ledger to the frame of the house above a lower level deck.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  (a) Front of Building - On or about February 3, 2016 no safe means of access/egress was provided for employees working on an upper level unfinished wood covering that was 11 feet 3 inches above an unfinished second floor deck and 26 feet 6 inches above ground.  Two employees were observed mounting a ledger to the frame of the house from the unfinished wood covering.
