29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: (a) South Dorm, Fifth floor - On or about February 4, 2016 an employee was working from the mobile scaffolding with the rollers not being locked in place to prevent movement. Then the employee was scooting him self along the floor to reposition the scaffold. Scaffold was located in front of the elevator doors and with dried plaster on floor.

29 CFR 1926.454(a)(3): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the proper use of the scaffold, and the proper handling of materials on the scaffold. (a) South building, third floor - On or about February 4, 2016 employee was working from a rolling scaffold had not been provided training on the use of said scaffold and how to recognize hazards around him while using said scaffold. .
