29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition: On 02/08/2016, Wunderlich Fibre Box Company, located at 821 Clinton Street, St. Louis, MO had two powered industrial trucks, (the Nissan 50 - the Co. Forklift No.: N-3, and the Tusk 300 forklift) were missing the LPG tank locator/indexing pins. Both were available for use and were being operated.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.: On 02/08/2016, an employee from Wunderlich Fibre Box Company, located at 821 Clinton Street, St. Louis, MO, was exposed to hazards such as those created by rotating parts. The Central Machinerys 3in1 Multipurpose Machine had its rotating chuck guard broken for the lathe portion of the machine. The guard was in a nonfunctional state.
