OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards because Below-the-Hook Lifting Devices were not tested and marked as required in ASME B30.20-2006, Section 20-1.2.1with the following: All new structural and mechanical lifting devices shall be marked with, but not limited to, the following information: - Manufacturers name and address - Serial number - Lifter weight, if over 100 lb. (45) kg - Cold current (amps) (when applicable) - Rated voltage (if applicable) - Rated load (as described in para. 20-1.2.1(a) - ASMS BTH-1 Design Category - ASME BTH Service Class (a) Precast/Pre-stressed Yard - On or about February 4, 2016, the Below-the-Hook lifting device on the MI-JACK MJ50 Travelift gantry crane was only marked with the load rating. There were no other required markings. (b) Precast/Pre-stressed Yard - On or about February 4, 2016, the Below-the-Hook lifting device on the MI-JACK 750D Travelift gantry crane did not have any of the required markings. Reference ASME B30.20-2006, Section 20-1.2.1 Marking; and Section 20-1.3.8 Testing
