29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: On or about February 9, 2016, on the south side of the building, where employees were exposed to fall hazards while being lifted to the roof in a platform that was not secured to the forklift.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about February 9, 2016, on the roof of the building, employees were exposed fall hazards of more than 15 feet while installing metal decking without a fall protection system.
