Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees were exposed to the hazards of flying shrapnel from an exploding plastic PVC pipe. The PVC pipe was used to transport compressed air throughout the shop. The piping ran from the air compressor up to the ceiling and then to strategic areas thought out the shop. The compressed air was at an operating pressure of 120 pounds per square inch (PSI). Among other methods, the following are feasible and acceptable methods to correction: 1. Replace the current PVC compressed air piping system with materials suited for the maximum air pressure as recommended by the Plastic Pipe Institute, such as ABS or metal. 2. Follow the American National Standards Institute/American Society of Mechanical Engineers (ANSI/ASME) B31.1-2007, Power Piping, as applicable.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet: In the maintenance bay area of the facility the employer is failing to ensure that oxygen cylinders are not stored within 20 feet of oil drums, tanks and containers. Various containers of oils and lubes were stored within 6 feet of the oxygen cylinders.
