29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:  During the week of January 18, 2016, the employer failed to ensure employees utilizing a supported tubular welded frame scaffold on rolling casters locked the casters prior to work. Employees utilized the scaffold to install new insulation in the work shop.

29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  On or about February 9, 2016, the employer failed to ensure employees operating a Mitsubishi propane forklift, model number FG30, serial number AF13D-31149 were certified. Employees utilize the forklift to load trucks with supplies on a daily basis.
