On 2/05/2016, employees from J. L. Heath Masonry, L.L.C. were installing veneer stone on the front of a commercial building located at 3257 County Road 211, Kingdom City, MO. Three employees were working from mast scaffolding approximately twenty feet above the ground. A portion of the scaffolding was missing guardrail systems. Two employees were working in the area and were not protected by personal fall arrest systems.

Heath Masonry, L.L.C. were installing veneer stone on the front of a commercial building located at 3257 County Road 211, Kingdom City, MO. Three employees were working from mast scaffolding approximately 20 feet above the ground. The employees were using a Genie Man Lift, Model S-45/Serial # 453660, to access the scaffolding. The employees were not wearing body belts with lanyards attached to the boom or basket while operating or riding in the aerial lift.
