29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) 27 Liberty Street, Dayton (So. Brunswick) NJ 08810. Employees were doing rip and tear of old roofing materials and installation of new roofing materials while not wearing any fall protection exposing the employees to fall hazards. Condition occurred on or about 02/02/16.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) 27 Liberty Street, Dayton (So. Brunswick) NJ 08810. Employees were using an aluminum portable extension ladder to access the roof when the ladder was no extended three feet above the landing exposing the employees to fall hazards. Condition occurred on or about 02/02/16.
