29 CFR 1910.1048(h)(1)(i): All contact of the eyes and skin with liquids containing 1 percent or more formaldehyde was not prevented by the use of chemical protective clothing made of material impervious to formaldehyde and the use of other personal protective equipment such as goggles and face shields, as appropriate to the operation:  a) In the embalming room, the employees did not wear eye protection while embalming, exposing employees to formaldehyde, a corrosive chemical.

29 CFR 1910.1030(d)(3)(xi): Appropriate protective clothing such as, but not limited to, gowns, aprons, lab coats, clinic jackets, or similar outer garments were not worn in occupational exposure situations:  a)  In the embalming room, the employees did not wear full body protection during the embalming process.  The protection provided did not cover the employees' arms, exposing them to bloodborne pathogens.

29 CFR 1910.1048(d)(1)(i): Employees of a workplace covered by this standard were not monitored to determine their exposure to formaldehyde:  a) In the embalming room, the employer did not conduct initial monitoring to determine the employees exposure to formaldehyde during the embalming process.     Murray Brothers Funeral Home was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.1048(d)(1)(i)], which was contained in OSHA inspection number 314644915, citation number 01, item number 006 and was affirmed as a final order on November 18, 2011, with respect to a workplace located at 1199 Utoy Springs Road, Atlanta, GA 30331.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication training did not include an explanation of the Global Harmonization System (GHS) with respect to the labeling, pictograms, and safety data sheets, including the order of information and how employees could obtain and use the appropriate hazard information:  a) Throughout the Facility - The employer did not provide training to employees on the new hazard communication standard Global Harmonized System (GHS) that took effect December 1, 2013. Employees were not familiar with the new hazard communication standard.  Employees were exposed to hazardous chemicals in the workplace, such as but limited to, formaldehyde, methanol, and glutaraldehyde.
