The back-up/warning light did not work when checked.      b.  The back-up alarm had been disconnected - confirmed by the owner.      c.  The machine was in use without having a seatbelt installed.

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):     All-American-Recycling, 25 Mohawk Drive, Warehouse #2, Leominster, Massachusetts; The employer recycles appliances with a variety of different types of Freon as well as handling electronics that have phosphorous and heavy metals.  They also use a variety of chemicals in the facility.  However, the employer had not created a hazard communication (HAZCOM) program.
