Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly:   a)     This violation occurred on or before February 9, 2016, in the warehouse where employee(s) exposed to powered industrial truck accidents, due to the employer modified powered industrial trucks by using a pallet for a personnel work platform without approval from the manufacturer.
