29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    On or about 10 February 2016, at this construction site, an employee was exposed to fall hazards while bricklaying without  the working level of the scaffold being fully planked.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound top rail capacity).    On or about 10 February 2016, at this construction site, an employee worked from a scaffold platform to access the work area at more that 20 feet to the ground level without a fall protection system.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  On or about 10 February 2016, at this construction site, an employee worked from a scaffold platform and used cross braces as a means of access to the work area at more that 20 feet to the ground level without proper fall protection.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.  On or about 10 February 2016, at this construction site, an employee worked on a scaffold  with small objects at a height greater than 20 feet above the ground level without containing or deflecting falling objects.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided.  On or about 10 February 2016, at this commercial construction site, the employer did not require all employees to wear personal protective equipment, hard hats, vests and safety glasses while working on a scaffold.
