29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) On or about February 10, 2016, employees were exposed to stuck-by hazards while working underneath second floor framing operations with no head protection (hard hats) in place.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about February 10, 2016, employees were exposed to fall hazards while installing floor joists and sub floor sheathing on the second floor with no fall protection in place. Fall distances ranged from 11 feet from the second floor to the first to 22 feet from the second floor to the basement; b) On or about February 10, employees were exposed to fall hazards where on the first floor, a floor hole was not covered. The fall distance to the basement level was 11 feet. c) On or about February 10, 2016, employees were exposed to fall hazards where guard rails were not in place at open sided floors above the basement level and at window openings. The fall distance from the first floor to the basement was 11 feet. The fall distance from the window opening to the ground was 10 feet.
