On or about February 05, 2016, at the balcony, employees were required to wear a Moldex 2200 N95 particulate respirator while performing forms work or chipping out bad concrete and the employer did not establish nor implement a written respiratory protection program that addressed the worksite-specific procedures such as but not limited to employee medical evaluation, fit testing to the employee, and training on the use, maintenance and care of the respirator.
