At the Milling Facility, the employer did not ensure that grain dust in the dryer basement was cleaned and removed from electrical equipment, machinery, and ledges exposing the employees to fire hazards.

29 CFR 1910.176(b): Storage was not secure. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. At the Grain Handling Facility Warehouse, the storage racks were not anchored or marked. Employees were exposed to falling objects and struck-by hazards created by potential structural failure and collapse of storage rack systems.: a. Storage racks located at the west side of the warehouse; b. Storage racks located at the east side of the ware house; and c. Storage racks located at the center on the south side of the warehouse.
