29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. At the Clyde ISD Elementary School job site in Clyde,Texas employees performing roofing activities on the gym's roof without fall protection while working around the skylights were exposed to fall hazards of approximately 18 feet to the ground below.
