29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a) 8730 Morrisette Dr., Springfield, VA 22152: On or about February 3, 2016, the employer did not ensure each employee had training and an evaluation prior to operating a Crown SC forklift and a Toyota forklift, exposing employees to struck-by, crushing and other hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
