29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:  At the jobsite located at 9444 E. 29th St. N., Wichita, KS 67226 (Towne Place Suites Wichita East): Employees engaged in roofing activities were exposed to fall hazards of approximately 17 feet from the eave of the residence to the ground below while accessing and exiting the roof in that the portable ladder did not extend at least 3 feet above the upper landing and was not secured to the structure.  29 CFR 1903.19(c)(2) abatement of the above violation was verified at time of inspection, no certification is required.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.    At the jobsite located at 9444 E. 29th St. N., Wichita, KS 67226 (Towne Place Suites Wichita East): Employees engaged in roofing activities from a 4:12 and 5:12 pitch roof were exposed to fall hazards of approximately 8 feet and 17 feet from the eave of the structure to the ground below in that fall protection was not utilized in the following locations:  a) Front porch by the entrance door  b) West side of the hotel roof c) Storage shed in the back of the hotel   The Mancinas Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 952615, citation number 1, item number 1 and was affirmed as a final order on 5/12/2014, with respect to a workplace located at 7911 E. Champions Ct., Wichita, KS 67226.  29 CFR 1903.19(c)(2) abatement of the above violation was verified at time of inspection, no certification is required.
