29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   (a) On February 11, 2016, at the facility located at 172 NE 1st LN, Lamar, MO., the employer had not developed an Energy Control Program ( Lock Out- Tag Out ) as required by this standard for the protection of employees from unexpected startup or release of stored energy. As result, employees are being exposed to serious injuries requiring medical attention and or hospitalization.   Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   (a) On February 11, 2016, in the Machine Shop area of the facility located at 172 NE 1st LN, Lamar, MO., an employee was exposed to caught-in injuries, in that there was no guarding provided for the milling machine drilling bit used to drill holes in small metal pieces. The employee was injured when his hand and glove were pulled into the rotating part of the machine which later resulted in a partial amputation of his left ring finger.    Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
