29 CFR 1910.305(g)(1)(iv)(C): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:  Where run through doorways, windows, or similar openings:  Instance #1:   Date of Violation: On or about 02-11-2016 and times prior thereto, located at Davis Boat Works, Inc., 99 Jefferson Avenue Newport News, VA 23607, in a shore side building near the brow of barge being worked on, employees were exposed to fire and electrical hazards from a 50 foot extension cord that was run through a personnel entry door opening whereas the door opened and closed down on the cord as it went out towards the outside gravel area to supply power to a handheld portable grinder.  The cord had obvious pinched deformities.   Instance #2:  Date of Violation: On or about 02-11-2016 and times prior thereto, located at Davis Boat Works, Inc., 99 Jefferson Avenue Newport News, VA 23607,  employee(s) were exposed to electrical hazards from welding cables passing through the water-tight hatch door on the naval berthing barge.   The cables had obvious pinch deformities with exposed conductor wires.  Abatement certification required within 10 days after abatement date. Instance #1:  Date of Violation: On or about 02-11-2016 and times prior thereto, located at Davis Boat Works, Inc., 99 Jefferson Avenue Newport News, VA 23607, onboard the naval berthing barge, employees were exposed to electrical hazards from a 50 foot extension cord that did not have any strain relief on the female end of the plug.  The cord provided power to a Dewalt brand DWE402 41/2 inch angle grinder.  Instance #2:  Date of Violation: On or about 02-11-2016 and times prior thereto, located at Davis Boat Works, Inc., 99 Jefferson Avenue Newport News, VA 23607, onboard the naval berthing barge, employees were exposed to electrical hazards from a ventilation fan that had missing strain relief where the electrical supply cord entered the fan housing unit.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
