29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) Bagging room, on or about 12-22-2015, employees used an Autobag HS-100 Excel bagger, with two Photo Optic Finger switches, located on the Plexiglas cover of the machine, where they were exposed to un-intended activation by the operator's forearms or clothing, exposing the employee to caught between hazards resulting in amputations.
