29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. (a) On February 11, 2016, at 9503 N Congress Avenue, Kansas City, MO 64153, An employee using a pneumatic impact gun to secure steel beams was not wearing eye protection. A second employee, operating the aerial lift, stood approximately 3 feet from this operation was also not wearing eye protection. The employees were exposed to eye trauma from flying objects.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) On February 11, 2016, at 9503 N Congress Avenue, Kansas City, MO 64153, an employee was exposed to a fall hazard of approximately a distance of 15 feet, in that he climbed out of an aerial lift and onto a steel beam. The employee detached his fall protection from the anchor point without reattaching to a new anchor point before climbing out of the lift.
