A second employee, operating the aerial lift, stood approximately 3 feet from this operation was also not wearing eye protection. The employees were exposed to eye trauma from flying objects.

(a) On February 11, 2016, at 9503 N Congress Avenue, Kansas City, MO 64153, an employee was exposed to a fall hazard of approximately a distance of 15 feet, in that he climbed out of an aerial lift and onto a steel beam. The employee detached his fall protection from the anchor point without reattaching to a new anchor point before climbing out of the lift.
