On or about Thursday, February 11, 2016 at 83-30 Vietor Avenue, Elmhurst, NY 11373 (Compactor Room A) An extension cord and a relocatable power tap were observed connected together in order to power a portable fan in lieu of permanent electrical wiring exposing employees to electrical hazards. ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
