In completed installations, each outlet box did not have a cover, faceplate, or fixture canopy:      a) Shop area; Westside wall: On or about February 11, 2016, and at times prior thereto, employees were exposed to electrical hazards while plugging in a portable radio to an outlet that had a missing cover plate exposing electrical conductors.         b) Shop Area; Westside wall: On or about February 11, 2016 and at times prior thereto, employees were exposed to electrical hazards while working near a cabinet with an open cover exposing electrical conductors.
