29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. a. Westway Terminal Company LLC, Philadelphia Terminal, Elevated pipe rack North side of Rail Area Loading Dock - The employer did not ensure that employees exposed to sodium hydroxide (corrosive) that dripped from an overhead valve or flange were provided with splash goggles and full-face shields. While an employee was applying fiberglass insulation onto a pipe, the employee sustained a second degree chemical burn to the face. Condition noted on or about 01/31/16.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59) a. Westway Terminal Company LLC, Philadelphia Terminal, Elevated pipe rack North side of rail area loading dock - The employer did not provide employees who were exposed to sodium hydroxide while applying fiberglass insulation onto pipes with information and training on the hazards of sodium hydroxide. An employee sustained a second degree chemical burn to the face. On or About 01/31/16.
