29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:   a) 40 Station Stone Lane, Litiz, PA; An employee was operating a Gradual Telehandler Model Number 534D6-42 without the use of a seatbelt, on or about February 12, 2016.   b) 40 Station Stone Lane, Litiz, PA; Employees were operating a Paslode Framing Nailer without the use of safety glasses while installing a dormer on a residential structure, on or about February 12, 2016.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks:   a) 40 Station Stone Lane, Litiz, PA; The employer did not meet the applicable requirements of operation when the operator of the Gradual Telehandler Model Number 534D6-42 did not lower the forks when he left the machine unattended, thereby exposing employees to a fall hazard of approximately 26 feet on or about February 12, 2016.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   a) 40 Station Stone Lane, Litiz, PA; Employees were installing a roof dormer while working from an elevated work platform on the forks of a Gradall rough terrain forklift. The work platform was not provided with standard guardrails, thereby exposing employees to a fall hazard of approximately 26 feet, on or about February 12, 2016.
