29 CFR 1926.451(d)(12)(i): When wire ropes were used on suspension scaffolds there was not a minimum of three wire clips installed a minimum of six rope diameters apart: On or about January 4, 2016 at the above addressed jobsite, employees were exposed to a 15-foot fall hazard when working from a swing stage scaffold that used a wire rope that was missing one clip.
