29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about February 10, 2016, at 2254 Henderson Mill Road, Atlanta, Georgia:  8 employees performing roofing work were exposed to struck-by hazards from falling roofing shingles and tools of the employees working above them while they worked at different levels of the roof and from the ground without using head protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about February 10, 2016, at 2254 Henderson Mill Road, Atlanta, Georgia:  8 employees performing roofing work were exposed to fall hazards ranging from approximately 8.9-feet up to approximately 25-feet and greater when they worked on an approximately 12/8 slope roof without using fall protection.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  On or about February 10, 2016, at 2254 Henderson Mill Road, Atlanta, Georgia:  Three employees performing roofing work were exposed to fall hazards ranging from approximately 8.9-feet to greater than 25-feet when they used rock climbing harnesses attached to life lines at waist level in the front.
