29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:     (a) On or about February 11, 2016, Roof Crafters, LLC., did not ensure that eye and face protection were used by employees when machines or operations presented potential eye or face injury. An employee of Roof Crafters, LLC., was observed nailing shingles on a residential roof, using a pneumatic gun, without proper eye or face protection.     Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) On or about February 11, 2016, an employee of Roof Crafter, LLC., was exposed to fall hazards of up to 24 feet, while installing a new roof without fall protection.    Abatement Note:  Abatement certification and documentation is required for this item. (see enclosed Certification of Corrective Action worksheet).    Roof Crafters, LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 831201, citation number 1, item number 1 and was affirmed as a final order on March 18, 2013, with respect to a workplace located at 1901 Dewhirst Dr. Colorado Springs, CO 80907.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) On or about February 11, 2016, An employee of Roof Crafters, LLC., was exposed to fall hazards of up to 11.5 feet while using an extension ladder that did not extend at least 3 feet above the upper landing surface nor was the ladder secured in place to prevent displacement.     Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").    Roof Crafters, LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 831201, citation number 1, item number 2a and was affirmed as a final order on March 18, 2103, with respect to a workplace located at 1901 Dewhirst Dr. Colorado Springs, CO 80907.
