29 CFR 1926.451(d)(3)(ix): Tiebacks were not secured to a structurally sound anchorage on the building or structure a) On or about 02/03/2016, at the above addressed work site, a tieback for a suspension scaffold was attached to the side of an eye bolt installed vertically on a wall and not secured to a structurally sound anchorage on the building exposing employees to a fall hazard of approximately 50-feet. b) a) On or about 02/03/2016, at the above addressed work site, a tieback for a suspension scaffold was attached to an eye bolt and anchor which was partially pulled out of the concrete and not secured to a structurally sound anchorage on the building exposing employees to a fall hazard of approximately 50-feet.
