29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:   a) At 9612  Challis Drive Pike Road, Alabama:  On or about February 9, 2016 and at times prior; the employer allowed his employees to use a skill saw on site that did not have a properly functioning guard which exposed his employees to amputation hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At 9612 Challis Drive Pike Road, Alabama:  On or about February 9, 2016 and at times prior;  the employer allowed his employees to install siding on a house while standing on a high slope roof without fall protection which exposed his employees to a fall hazard of 10.7 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At 9612 Challis Dive Pike Road, Alabama:  On or about February 9, 2016 and at times prior; the employer allowed his employees to use an extension ladder to gain access to the roof top and the rails of ladder did not extend at least 3 feet past the landing area which exposed his employees to a fall hazard of 10.7 feet to the ground below.
