29 CFR 1910.138(a): General requirements. Employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes.    a. On or about February 29, 2016, employees were required to work with and around "MSP Releasing Agent" in the performance of their assigned duties. The employer failed to select and provide adequate gloves to protect the employees from skin absorption.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).    Production area - On or about July 15, 2016, employees were required to operate powered industrial vehicles in the performance of their assigned duties. The employer failed to ensure that all employees were adequately trained and evaluated to ensure safe operation, prior to operating such equipment. Employees were thereby exposed to struck-by hazards.    Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation that it is in compliance with this standard.

29 CFR 1910.179(d)(4)(iii): Ladders shall be permanently and securely fastened in place and shall be constructed in compliance with 1910.27.     Production area - On or about July 15, 2016, employees were required to work in and around a Mi-Jack Travelift gantry crane, in the performance of their assigned duties. The employer failed to ensure that a ladder meeting 1910.27 requirements was provided for access to and from the equipment. Employees were thereby exposed to fall hazards from approximately 10 feet with each entry / egress of the cab.    Pursuant to 29 CFR 1903.19(d), within ten (10) calendar days of the abatement date, the employer must submit documentation that it is in compliance with this standard.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.    a. On or about February 08, 2016 an employee suffered a struck-by / crushing injury and subsequent hospitalization.  The employer failed to report this incident to OSHA within 24 hours, as specified in this standard.

The employer failed to ensure that all employee certifications reflected the training received specific to the equipment operated as outlined in paragraph (l) subsection (3)(i)(M)    Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).
