29 CFR 1910.23(c)(1): 29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3) of this section) on all open sides: a) Core Connections Area - On or about February 16, 2016, while connecting wiring cores to transformers, employees walked/worked on top of a Shangdo platform that was not guarded on all open sides, thus exposing employees to a fall hazard of approximately 7 feet onto concrete flooring.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Area 11 Warehouse, Forklift Battery Charging Area - On or about, February 16, 2016 and at times prior to, forklift operators used an Exide Technologies Industrial Battery Charger (S/N: 09N9861M) that was not installed and used per the manufacturer's safe operating procedures, in that the battery charger was mounted on combustible material such as a wooden pallet, exposing employees to a fire hazard.
