29 CFR 1926.502(d)(20): The employer did not provide for prompt rescue of employees in the event of a fall or did not assure that employees were able to rescue themselves: a) Jobsite - Employees engaged in fuel storage tank insulation were exposed to a fall hazard greater than forty seven (47) feet above the grade. The employer did not provide a rescue plan to employees in the event of an emergency or the need to self rescue as a result of the fall hazard exposure. Noted on or about 10 February 2016.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Jobsite - Employees engaged in fuel storage tank insulation were exposed to a fall hazard greater than forty seven (47) feet above the grade without fall protection. The employer did not utilize a personal fall protection system, guard rails or a safety net system to protect employees from falling. Noted on or about 10 February 2016. The Storage Tank Insulation Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 314981242, citation number 1, item number 2A and was affirmed as a final order on 13 January 2012, with respect to a workplace located at 33 Hubbels Drive, Mount Kisco, New York 10549.
